Reminders for All Student-Athletes:
Returning Student Athletes
Ø Fill out ALL paperwork under the “Returning Student-Athlete Paperwork” link on the main page
Ø Get results or provide documentation concerning Sickle Cell status à NCAA regulation
Ø Review Policy and Procedure Manual for new or changed policies
New/Transfer/First Time Varsity Athletes
Ø Fill out ALL paperwork under the “New Student-Athlete Paperwork” link on the main page
Ø Get results or provide documentation concerning Sickle Cell status
Ø Review Policy and Procedure Manual
Ø Make sure shot records, TB test/results, and Tetanus shots and records are submitted to the Student Health Services
Ø Make sure any pre-existing conditions general medical issues or orthopedic issues that are either chronic or within the last two year have documentation from the primary care physician treating the condition. Send these with your history paperwork.
